Lesson Policy Agreement

IMPORTANT: Parents or Guardians need to complete and submit the lesson policy agreement below. If you are a student please stop and have a parent or guardian complete the form below.

After Mrs. Powell receives your lesson policy agreement, she will send your invoice (payable online) for the first month's tuition payment. Please note: To ensure both of us are committed to making lessons happen, Mrs. Powell cannot hold a spot in her schedule for your student until the following is complete:

First monthly tuition payment is complete OR an autopay enable card is set up on your My Music Staff account

Additionally, if you are new to Mrs. Powell's studio, she will send you a separate email to create your password to access your free My Music Staff account after you submit the lesson policy agreement. My Music Staff is Mrs. Powell's billing and scheduling software for music lessons.
